Jim was kind enough to send this picture from his survivor H. 

Note that the washer is directly under the eye bolt head and not lower on the assembly - (around the threaded portion). 

I believe that this installation allows room for the carpet and pad and that the "shoulder" of the top eye portion makes direct contact with the floor pan.

My next question is, are they painted or just zinc plated? 

Also, do the washers fit around the threaded portion or around the larger diameter of the eye piece that is attached to the threaded portion?

Doug, Yes.  My Survivor H has them and they appear to be 1.5 inches in diameter.
