I got one several years ago from NAPA and it included everything but the  
housing that the "u-joint" fits inside of.  
 
Word of caution:  The "cross-pin" can be a royal pain to get  out.  The 
machine shop I used maxed out their press trying and never did  get it out.  Also 
ended up bending the pin slightly. 
 
You might want to consider leaving the pin in.  You can work the boot  over 
the pin but it takes a lot of patience.  I ended up poking a hole in  mine 
while doing it.  And any holes will ultimately lead to the boot  ripping itself 
apart.
