My next question is, are they painted or just zinc plated? 

Also, do the washers fit around the threaded portion or around the larger diameter of the eye piece that is attached to the threaded portion?

Doug, Yes.  My Survivor H has them and they appear to be 1.5 inches in diameter.

Is there supposed to be a washer between the eye bolt head and the carpet? 

If so, does anyone know the size?
