I saw that you had already sorted out your issue, but I thought
I'd post these pics just the same in case  someone else is looking into
this...

this is what I did for my "inside" adapter, the one that bolts to
the bellhousing (bh).,  my z-bar was cockeyes and not straight,
so I positioned it,  made a bracket out of cardboard that held the z-bar
straight, and positioned the fork pushrod straight.,,, and then
made one out of some stock and thick washers... 
I also fabbed a V8 z-bar from a \6 z-bar...  also in pics..
