All: I agree totally with Joe Savard about the quality of the fasteners (bolts and washers) you got.  From my past work experience, I learned that the US is flooded with counterfeit fasteners. What makes them counterfeit is that they are made in a foreign country out of scrap metal of unknown quality and marked as grade 5 or 8 fasteners.
 
    How do you get good fasteners?  I don't know for sure, but try to determine if your supplier will stand behind his products, or buy from reputable supplier.

Assuming that you assembled everything correctly, the only thing that I can think of is that you got some of the wrong grade bolts/washers.  When I was a Department of Defense inspector, we had a series of cases where bolts were wrongly marked, and failed.  They were too soft.
